Transition to Adulthood for young people with learning difficulties and/or disabilities in the south east of England

On 1st and 2nd November 2007, representatives from local authorities across the south east came together with representatives of the recently formed Strategic Transition to Adulthood Group.  The workshops aimed to begin the process of supporting every local authority to develop a robust transition strategy including

      • multi agency working
      • person centred transition planning
      • improving post 16 opportunities

in order to achieve better outcomes for young people.

The workshop report and copies of the presentations provide an insight into current initiatives and good practice in the region and show how the work will be taken forward.
